üThe name and phone number of an emergency contact who lives locally.

üPhone number(s) where I can be reached.

üThe name, phone number, and vehicle description of any person with access to my home.

üDo I want the sitter to recycle glass, cans, paper, cardboard, or other materials?  Where?

üAre there any special instructions about the animals, the plants, or the house?

üDo I have a leash & collar/harness for each dog; a halter/collar & lead line for each horse, lama, goat, other large animal; a carrier for each cat or small animal?

üThe name and phone number of a contact and the arrangements to be made in case of the sudden death of a pet.

üThe name and phone number of a contact and the arrangements to be made in the event of my inability to return from my trip.
